Once you sign-up for our block fee service, you’ll have unlimited access to:
Unlimited prescription renewal requests via phone, email and fax with medication review
Ability to contact our office via email for appointment requests and administrative questions
Sick or return to work notes
Sick or return to school notes
School, daycare, and camp medical forms
Travel consultation (vaccination cost in addition)
Uninsured vaccination administration (vaccination cost in addition)
Review of requests for Allied Health Services (Physiotherapy, Chiropractic, Massage, Orthotics, Assistive devices, etc.)
Tuberculosis (TB) test and reading, including 2-step
Provision of printed or electronic medical notes or test results
50% reduction in Driver’s Medical Examination and report
50% reduction in legal and insurance form completion

The Ontario Health Insurance Plan (OHIP) pays for most of your medical visits to the office. However, there are many time-intensive, uninsured services for which OHIP does not compensate family physicians. Some of these services include emails, prescription renewals over the phone or fax, sick notes, forms, and TB tests. As opposed to a pay-for-service basis, we suggest subscribing to the valuable Annual Fee Program. This program is optional and choosing not to participate will not impact your access to care.

Individual Plan: $150.00/year
Couple: $200.00/year
Family Plan: $250.00/year
Senior (65+) Individual Plan: $120.00/year
Senior Couple Plan: $150.00/year

All family members must live in the same household
Family plan includes all children 18 and under and full-time students
